For example, while the Institute for Health Metrics and Evaluation estimates that malaria causes more than 14% of all deaths in Niger, PMI Measure Malaria’s estimate charges malaria as the cause of more than 50% of the country’s deaths.

Countries With the Highest Malaria Death Rates (per 100K)
As of 2023 data – the most recent data available in early 2026 – the highest malaria death rates were overwhelmingly concentrated in Sub-Saharan Africa, reflecting a combination of high transmission, limited healthcare access, and persistent poverty.
Sierra Leone recorded the highest estimated malaria death rate in the world at 131 deaths per 100,000 people, followed closely by Niger (116) and Burundi (90.8). Several West and Central African countries also reported extremely high rates, including Benin (89.2), Nigeria (88.5), Burkina Faso (87), and the Central African Republic (84.9).
Countries With the Highest Total Malaria Deaths
Looking at total malaria deaths rather than death rates shifts the focus toward population size and overall disease burden. In 2023, countries with large populations and widespread malaria transmission accounted for the vast majority of global malaria deaths.
Nigeria recorded the highest estimated number of malaria deaths worldwide, with approximately 216,700 deaths, far surpassing any other country. The Democratic Republic of the Congo followed with about 75,000 deaths, while Niger reported roughly 30,900 deaths. Several other countries also experienced substantial losses, including Cameroon (28,200), Ivory Coast (27,300), and Uganda (25,400).
